Strange hacks/mods

Has anyone ever been playing on a game online and realised that some of the gamers are cheating or using mods? I was recently playing Turf War on GTA 4 and realised that the middle turf at Bohan was completely sealed off! I also noticed that the people on the other team were riding around in cars that you simply cannot get in the game. There were many strange occurrences in this game and I have also noticed this before. So my question is, how do they do this? are they loading up a CD with the mods on before starting the game or what? and have you ever noticed anything similar to this whilst playing games online?

They have their consoles modded most of the times, but i think that doesn't work for most of the mods that exist because those don't allow you to go online.

Oh okay, I never thought that it would be a physical mod, that seems quite strange actually. I don't see why someone would get a physical mod to change GTA online, but I suppose it hacks other games too?
