Disc Cannot Be Read.

I cannot even start a multiplayer match on call of duty IW or MWR without my PS4 saying it cannot read the disc, close the application and eject the disc. I can't play the game at all. Anyway to fix this? I've tried rebuilding database, etc...

Much probably (and hopefully) you disc reader lens is dirty.

The only way to clean it safely is with one of those lens cleaners, like the following one:

Try Alex's idea first. Could be a dirty lens. If that doesn't work, it's likely a faulty laser. I've owned 4 generations of PlayStation consoles, 2/4 had disc read errors from a bad laser. Bought a new PS2 when it happened and replaced the laser on my PS3. If the cleaner doesn't work you have 3 options. A) send it in for repair if under warranty or pay them if it's not. B) buy a new console. 3) replace the laser yourself. I fixed my PS3 for 25 bucks and I imagine it costs the same on the PS4.
